Accessibility checker found 23 images with missing alt text on my e-commerce site. Fixed them all with descriptive text. Google started indexing the images properly in image search within 2 weeks.

Colour contrast failures are the most common issue I see flagged. A lot of themes use light grey text on white backgrounds that fails WCAG AA. Easy to fix in CSS but easy to miss without a checker.

Missing form labels are another common one. Adding proper label elements also improved my form conversion rate — turned out users were confused too.
