Went from 41 to 78 on mobile by doing three things: enabling Cloudflare caching, converting all images to WebP, and deferring Google Fonts. Each one made a measurable difference.

The render-blocking resources flag is always the first thing I tackle. Moving scripts to the footer or adding async/defer attributes is usually the quickest win.

My score tanks on mobile but is fine on desktop. Anyone else seeing this pattern? Is it mainly image sizes?

Very common. Mobile issues are usually: large images not being resized for small screens, render-blocking third party scripts, and no lazy loading. The tool flags all three — work through them in order.
